Change of address

Marriage cert, divorce decree, Certified court order
Change of name: Corporation or company
State-issued certificate of name change, Copy of tax ID issued
Power of attorney or attorney-in-fact
Executed and notarized aif/poa
Conveyance or transfer of interest
County recorded conveyance Document, where the property is located
Divorce

Conveyance into trust
Copy of trust or trust certificate, recorded conveyance document transferring property to the trustee of a trust
Termination of trust
Documents required by you: Copy of trust dissolution, recorded conveyance to a beneficiary
Change in trustee
Copy of trust or trust certification supporting the successor trustee or declination of trustee
Distribution of estate
Owner’s death in the same state as the property is located
– death certificate, order admitting will to probate, letters testamentary, last will & testament, final distribution document via executors deed, final decree or judgement of possession, all made of record in the county/parish where the property is located
Owner’s death in a different state from where the property is located
– death certificate, order admitting will to probate, letters testamentary, last will & testament, proof of ancillary probate in the state where the property is recorded, affidavit of heirship if applicable (affidavit of heirship)
